Used by: List-Unsubscribe header + {{Unsubscribe_URL}} token in every Phase email
URL pattern:

What it does:
1. Reads email + source query params from URL
2. POSTs to Make.com unsubscribe webhook (Hook 2387225) → writes to Data Store 103952
3. Shows branded confirmation
4. Allows optional "reason" input (helps refine future content)
Deployment:
In InsuranceSplash editor:
1. Create new page at slug /unsubscribe (do NOT hide from nav — must be publicly reachable per CAN-SPAM)
2. Add an HTML widget
3. Paste everything below into the widget
4. Save + Republish site
5. Test: visit https://www.tapinsuretx.com/unsubscribe?email=test@example.com&source=test
6. Confirm Data Store 103952 picks up the test record
